Summary: We need general Rad Onc coverage to support our team, we anticipate this locum to help with both inpatient and outpatient coverage along with being on call once a month. RATE CAPS FOR SUBMISSION: Malpractice Must be included in rates. Orientation $190 - half the hourly/callback rate, kicks in after 8 hours. Per hour of orientation or training beyond 8 hours Hourly $380 - Per hour, not OT Holiday $570 - Must be 1.5x the hourly rate. Per holiday hour, no OT Nightly Call $450 - Per night call to hold the pager, to include 2.5 hours gratis Holiday Nightly Call - $675 - "Per night call to hold the pager, to include 2.5 hours gratis On Call (Daily) $2100 - Per 24 hours to include 4 hours gratis" 24 Hour Holiday Call Rate - $3150 Per 24 hours to include 4 hours gratis" Call Back $380 Per hour of patient contact hours, after gratis hours are completed, gratis prorate to MSA terms. Holiday Call Back $570 Per holiday hour of patient contact hours, after gratis hours are completed, gratis prorate to MSA terms. Schedule/Availability Requirements: 9/15/2025-1/1/2026 Facility Location: Dignity Health Cancer Institute at St. Joseph's Hospital and Medical Center Setting: supports two locations (one hospital based, one outpatient based) 1.5 miles apart; this person will have a significant role in helping manage the inpatient referrals and management primarily at the hospital based location Duties: provide consultation services for steady volume of inpatients referred for radiation treatment; provide consultation services for outpatient practice supporting 3 full time providers; supervising physician for linac based outpatient treatment; treatment planning via Eclipse; check daily imaging; provide OTV services; supports two locations (one hospital based, one outpatient based) 1.5 miles apart; rotate with 3 full time providers to take on call services approx 1 week per month (covering inpatient treatments, emergent consultation/sim/tx services, paged by answering service for patients calling answering service outside business hours for emergent needs (medication managment, symptom triage, etc) Days per month: full time; Monday - Friday; on call duties 1 week per month (on call is 7 days per week (weeknight evenings 5:00 pm-7:30 am; Saturday/Sunday 24 hours) Required Procedures: provide linac based treatments on Varian TrueBeam linear accelerator (3d, IMRT, SBRT); consultation, treatment planning, treatment supervision, OTV patient management, follow-ups EMR:ARIA, Cerner Support Staff: 3 Rad Oncs, 1 Practice Manager, 4 RNs, 3 MAs, 4 Medical Physicists (1 Chief of Physics), 2 Dosimetrists, 6 Radiation Therapists, front/back office support staff, procedure scheduler. Reason for Coverage:full time provider resigned position, actively recruiting to fill the position Deal breakers:needs to be able to take on call services 1 week per month Travel Preferences: Preferred Lodging: Preferred hotels? Air Travel: Local, no flights? Please detail any anticipated travel exceptions, for example a dietary restriction necessitating specific lodging situations. Travel exception requests will be denied if this information is not part of the provider presentation. Please also indicate the city where provider will be traveling from in the travel section of the provider submission. Credentialing Information: Timeframe: 90 days for new provider to the system Requirements: AZ State License or IMLC in hand Credentialing with other facilities? No Billing enrollment - Is this position providing services in lieu of an existing enrolled provider for a period of 60 days or less ? No Additional Details / Questions Answered by Facility: Admissions per day, Patients/day, Types of cases, Disclosures that may need to be made, etc. Patient load varies. Average inpatient consultations/ on treatment volume is 4 patients per day, but can range from 1-8 per day. On treatment outpatients at outpatient location average ~25-30. Thyroid, H&N, thoracic, GI, GU, sarcoma, melanoma, lymphoma, metastatic disease.
